Vampirism añade vampiros y cazavampiros al juego, además de permitirnos convertirnos en un vampiro, lo que nos aportará nuevas habilidades, más poder de ataque y un aumento de la salud. El mod añade un nuevo bioma al juego, habitado por varias clases de vampiros, así como cazavampiros que intentarán acabar con los vampiros, incluso con nosotros cuando decidamos convertirnos en vampiro.
Para convertirnos deberemos dejar que un vampiro nos muerda. Nos daremos cuenta de que estamos infectados por qué aparecerá un nuevo efecto y partículas alrededor de nuestro personaje. Pasado un tiempo nos convertiremos. Existen varios niveles de vampiro, así que deberemos realizar rituales de sangre para ascender nuestro nivel de vampiro, desbloqueando nuevas habilidades, como por ejemplo convertirnos en murciélago y volar en supervivencia, entre otros.
En el bioma de los vampiros se encuentra en castillo de Drácula, el señor de los vampiros. Pero sólo podremos entrar al interior del castillo cuando consigamos el último nivel de vampiro. Entonces podremos enfrentarnos a Drácula en una nueva dimensión y así convertirnos en el señor de los vampiros.
