Vampirism nos presenta un mundo con nuevos habitantes, muy poco amistosos. Al instalar el mod nos encontraremos con un mundo lleno de vampiros y otras criaturas hostiles deambulando por el mundo, aunque tienen un bioma propio donde aparecen con mayor frecuencia. En este nuevo bioma también se generan poblados, e incluso el castillo del Conde Drácula, al cual podremos acceder cuando nos hayamos convertido en vampiro y tengamos cierto nivel.
Para empezar tendremos que convertirnos en vampiro y para ello deberemos dejar que nos muerda un vampiro. Una vez estemos infectados, podremos realizar rituales de sangre para subir nuestro nivel de vampiro, consiguiendo poderosas e interesantes habilidades por el camino, como por ejemplo convertirnos en murciélagos para poder volar, y en última instancia pudiendo acceder al castillo del Conde Drácula.
Una vez consigamos entrar al castillo del señor de los vampiros ya sólo nos quedará derrotar a Drácula para así arrebatar su título de señor de los vampiros, y conseguir un montón de habilidades espectaculares y poderosas.
